Pacific Edge Diagnostics New Zealand Limited (referred to as "Pacific Edge,""we" or "us" in this Privacy Policy) is committed to maintaining the privacy of your personal information provided to us as a consequence of you ordering testing services from us.

This Privacy Policy explains how we may collect, store, use and disclose such personal information (including health information).

What personal information do we collect or store

    1. We may collect personal information about you from time to time as result of you ordering testing services, including your:
      1. name
      2. date of birth
      3. address (postal and email)
      4. gender
      5. telephone numbers
      6. NHI number
      7. health professional(s)
      8. test results and other health information
      9. other information necessary to perform our services and other functions and activities.

Collection of personal information

    1. We may collect personal information about you:
      1. directly from you when you order our testing services or when we perform these testing services,
      2. when you contact us to update personal information that we hold about you,
      3. from your medical practitioner or another health professional, and
      4. from any other person with your authorisation or where otherwise permitted by law (including the Privacy Act 1993 or the Health Information Privacy Code 1994).

Use and disclosure of personal information

    1. We will not use or disclose your personal information except in accordance with privacy law (including the Privacy Act 1993 or the Health Information Privacy Code 1994). We may use or disclose your personal information for the purposes of:
      1. providing you or your representative with services and information regarding our testing services,
      2. communicating with you or your representative,
      3. communicating with, or providing information to, your medical practitioner or other health professional,
      4. communicating with your health insurance provider,
      5. providing you with any information requested by you,
      6. gauging customer satisfaction and providing customer services,
      7. improving our information or services,
      8. maintaining our records,
      9. providing technical support or administration,
      10. addressing feedback or complaints and resolving disputes,
      11. providing you with promotional materials and news, where you have agreed to receive such information,
      12. legislative and regulatory compliance,
      13. enforcing the terms and conditions governing the testing services we provide to you,
      14. billing and debt recovery, and
      15. complying with Payment Card Industry Data Security standards (PCI DSS) with respect to any credit card you use to pay for our testing services .

Holding personal information

    1. We will not hold your personal information for longer than is reasonably required for the purposes for which we may lawfully use that personal information.

Your rights to access and correct personal information.

      1. Where we hold personal information about you, you have a right to access and correct that personal information in accordance with applicable privacy laws. If you want to access or correct your personal information, please contact our Privacy Officer (see paragraph 10 below) who will tell you how to make a request.
      2. Despite this, we will not provide any test results directly to you. You may ask your medical practitioner or other health professional for a copy of your test results, if you wish.

Security

      1. We will use all reasonable endeavours to effect and maintain adequate security measures to safeguard the personal information we hold from loss or unauthorised access, use, modification or disclosure.  However, as the internet is an open network, we do not warrant or guarantee that your personal information is safe from loss or unauthorised access, use, modification or disclosure, either during transmission to or from us, or when held by us. We will dispose of personal information in accordance with privacy law (including the Privacy Act 1993 or the Health Information Privacy Code 1994).

Amendments

      1. We may amend this Privacy Policy from time to time.  Notice of any such amendments will be announced on our website and will be effective immediately, unless we state otherwise.  You are responsible for reviewing our website regularly to obtain timely notice of any such amendments.
      2. If you continue to order testing services from us or if you provide personal information after we post changes to this Privacy Policy, this will constitute your acceptance of such changes to this Privacy Policy.
